Cant get my from to send e-mail

Hi there,

I am working with dreamweaver cs3 for Mac
I have managed to sucessfully use the php test page from mind palette to send me an e-mail so that all is ok with the PHP script.
So then I follow the PDF instructions for deamweaver, add my e-mail address in the code. Then in dreamweaver select the table tag & change appropriate info, then uploaded to my remote server. Filled out my form, clicked send & then I get a page saying "Not Found
The requested URL /form.php was not found on this server."
This file exists but don't know how the hell I managed to link my Form.html to Form.php!
So I get this message and no e-mail sent to my account.

Do you know why?
