Process Form 3 Troubles

I've installed Process Form 3 using Dreamweaver CS3. I followed the supplied instructions( which were nicely illustrated and thoughtfully written) but still cannot generate a successful confirmation page or an email. I've uploaded the PHP test and received an email, so the server is working as it should. After I click submit, I get an empty page. PHP Mail is enabled on the server side(Yahoo) as is MYSQL, though a quick check of the SQL logs shows no process attempt. Any thoughts?
